Job Description

We are looking for a dedicated Chronic Care Navigator to join our team at Phamily. As a Chronic Care Navigator, you will play a vital role in supporting patient-centered, team-based care in the field of nephrology. Your responsibilities will include utilizing the Phamily platform to manage patient data, identify unmet needs, engage patients in their healthcare, and enhance communication between patients and their care teams.

Key Responsibilities

• Understand nephrology requirements for optimal coordinated population health.
• Collaborate with care teams to establish effective care processes.
• Utilize the Phamily platform to support patients with chronic diseases and coordinate care.
• Contribute to quality improvement and care redesign efforts.
• Provide support to develop interventions for managing target populations.
• Ensure a positive experience for patients through courteous interactions and proper data management.
• Attend meetings and participate in teamwork within and between departments.

Minimum Qualifications

• At least 1 year of experience in nephrology.
• Strong problem-solving skills and ability to multitask.
• Excellent written and verbal communication skills.
• Certified Medical Assistant from a recognized organization.
