INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Case Manager, you will collaborate with the clinical team and medical providers to deliver exceptional discharge planning coordination and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, enhancing care outcomes, optimizing health service utilization, and reducing payment denials.

Responsibilities

• Coordinate discharge planning efforts in conjunction with the clinical team and medical providers.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment for patients with complex health needs.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• BSN required from CCNE accredited schools.
• Prior experience in a similar role is advantageous.
• Strong interpersonal and communication skills.
