Job Description
We are looking for an experienced Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Case Manager, you will collaborate with the clinical team and medical providers to deliver discharge planning coordination and interventions. The role focuses on promoting patient wellness, improving care outcomes, ensuring efficient utilization of health services, and reducing payment denials for patients with complex health needs.
