Care Manager

Job Description

Sunrise Senior Living is seeking a compassionate Care Manager to join our team in providing quality care and services to residents in our assisted living and reminiscence neighborhoods. As a Care Manager, you will build meaningful relationships with seniors and their families, attend to individual care needs, and ensure a high standard of service.

Responsibilities

• Provide assistance with activities of daily living and attend to individual care needs.
• Notify management of changes in resident conditions and recommend appropriate adjustments.
• Participate in developing Individualized Service Plans (ISP) and adhere to safety regulations.
• Ensure residents' dining needs are met while upholding Sunrise service standards.
• Engage residents in life enriching activities and maintain a caring and supportive environment.

Qualifications

• Dedication and passion to serve seniors with excellent customer service skills.
• Positive attitude, flexibility, and ability to work collaboratively in a team setting.
• High School diploma/GED required; certifications may be necessary per state regulations.
• Previous experience working with seniors is preferred.
• Effective communication, organizational skills, and good judgment are essential.
