In this role, you will be responsible for participating in the Resident Assessment Process by completing items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure appropriate patient care and center reimbursement.

Position Highlights

• Direct the RAI process, ensuring compliance with guidelines.
• Develop individualized resident Care Plans.
• Train interdisciplinary team members on clinical content and MDS completion.

Requirements

• LPN in good standing with state license.
• Prior experience in healthcare facility settings.
• Proficiency in general and rehabilitative nursing practices.
• Familiarity with MDS completion and Care Plans.
