Job Description

Join Superior HealthPlan's Medical Management team as a Care Manager RN - Foster Care and make a difference for our members. As a licensed professional residing in Texas, you will perform care management duties to assess, plan, and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost-effective care.

Responsibilities

  • Assess the member's current health status, resource utilization, treatment plan, and services
  • Develop a plan of care based on assessment with specific objectives, goals, and interventions to meet member's needs
  • Coordinate services between medical providers and non-medical staff as necessary
  • Facilitate member access to community-based services and monitor referrals
  • Participate in integrated team care management rounds
  • Identify and report related risk management quality concerns
  • Enter and maintain assessments, authorizations, and clinical information into medical management systems
  • Coordinate overall care management activities and review and approve the initial and ongoing assessment and care plan
  • Provide patient and provider education and participate in case management committees
  • Assist members and/or agency staff with transportation to healthcare appointments
  • Communicate status of member health indicators with agency staff
  • Participate in weekly rounds with agency staff
  • Perform other duties as assigned

Education/Experience

Graduate from an Accredited School of Nursing. Bachelor’s degree in Nursing preferred. 2+ years of clinical nursing experience and 1+ years of case management experience in a managed care setting. Knowledge of utilization management principles, healthcare managed care, and medical decision support tools. Current state’s RN license is required.

Texas Requirements

Graduate from an Accredited School of Nursing. Bachelor’s degree in Nursing preferred. 2+ years of clinical nursing or case management experience in a clinical, acute care, managed care, or community setting. 2+ years experience working with people with disabilities and vulnerable populations. Current state’s RN license is required.

Pay Range

$66,100.00 - $118,900.00 per year
