Care Manager RN - Foster Care

Job Description

Superior HealthPlan is seeking a Care Manager RN for Foster Care. As a member of our Medical Management/Health Services team, you will assess, plan, and coordinate all aspects of medical and supporting services for select members, promoting quality, cost-effective care. This hybrid position requires residing in Texas and pediatric experience.

Responsibilities

• Assess member's health status, treatment plan, and services, developing plans based on assessment to meet member's needs and promote desired outcomes.
• Coordinate services between PCP, specialists, and non-medical staff.
• Provide patient and provider education, facilitate member access to community-based services, and monitor referrals made to support members’ care management plan.
• Participate in integrated team care management rounds, identify related risk management quality concerns, and report scenarios to appropriate resources.

Education/Experience

Graduate from an Accredited School of Nursing, with a Bachelor’s degree in Nursing preferred. Requires 2+ years clinical nursing experience and 1+ years of case management experience. Knowledge of utilization management principles and healthcare managed care. Current state’s RN license.
