Case Manager - Hennepin County

Job Description

We are looking for a passionate and dedicated Case Manager to join our Hennepin County team at Pinnacle Services. As a Case Manager, you will have the opportunity to work with individuals on CADI, DD, or AC/EW waivers, assisting them in various aspects of their lives. Responsibilities

• Assess areas of the persons served life requiring assistance.
• Offer and coordinate resources for the persons served.
• Monitor service delivery and ensure needs are met.
• Develop service plans and attend meetings with interdisciplinary teams.
• Document client work for billing purposes.

Requirements

• Bachelor's degree in social work, psychology, sociology, or related field.
• Valid driver's license and reliable transportation.
• Clear background checks.
• Experience in social work or case management is desirable.
• LSW or BOSW Licensure preferred.
