CSC Case Manager

Job Description

PermiaCare is seeking a dedicated CSC Case Manager to join our team in Midland. The CSC Case Manager will work with individuals aged 15 to 30 experiencing their first episode of psychosis, providing psychoeducation, psychosocial rehabilitation, and case management services. This role involves collaborating with a multidisciplinary team to support individuals in employment or education needs and mental health recovery.

Responsibilities

• Provide direct services and support to individuals in the CSC-FEP program.
• Develop and implement comprehensive recovery plans in alignment with individual needs.
• Coordinate services and link individuals to necessary resources for holistic care.
• Ensure documentation compliance and maintain confidentiality of sensitive information.
• Participate in community outreach and education initiatives as directed.

Requirements

• Bachelor's degree in psychology, social work, nursing, or related field.
• 1 year of experience in social services; QMHP certification within 6 months of hire.
• Valid Texas Driver's license, clear driving record, and ability to pass background checks.
• Knowledge of mental health treatment, HIPAA regulations, and trauma-informed care.
