Job Description
We are looking for a dedicated and compassionate Inpatient Case Manager (RN) to join our team at LifeBridge Health. As a vital member of the healthcare team, you will collaborate with clinical staff and medical providers to coordinate discharge planning and interventions for patients with complex health needs.
Responsibilities
- Provide discharge planning coordination in collaboration with the clinical team.
- Promote patient wellness and improved care outcomes.
- Ensure efficient utilization of health services.
- Minimize denials of payment among a patient population with complex health needs.
Requirements
- Bachelor's degree in Nursing (BSN) from CCNE accredited schools.
- Professional knowledge of theory and practice in a specialized field.
- Experience as a Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager at LifeBridge Health is preferred.
