Post Permanency Coordinator

Job Description

We are looking for a Post Permanency Coordinator to join Asociación Puertorriqueños en Marcha (APM) in Philadelphia, PA. As a key member of our team, you will be responsible for ensuring compliance with Adoption State and County regulations and providing case management services to children, youth, and families referred through the Statewide Adoption and Permanency Network (SWAN).

Responsibilities

• Assess the physical condition of resource homes.
• Complete Post Permanency Services according to SWAN guidelines.
• Monitor and document service progress.
• Maintain updated files and provide status updates on families/services.
• Assist with training and social work services.
• Attend necessary meetings, conferences, and court sessions.

Requirements

• Bachelor's Degree in Social Work, Psychology or related field.
• Minimum one year experience in adoption or social service delivery.
• Familiarity with PA Chapter 3700 and Microsoft Office Suite.
