Job Description

We are seeking a Certified Social Worker with a Master's Degree in Social Work and clinical expertise in providing quality patient care. Responsibilities
- Assess patients' and families' psychosocial, environmental, and financial needs.
- Develop, implement, and evaluate personalized care plans.
- Collaborate with patients, families, and interdisciplinary teams.

- Maintain accurate documentation records.
Requirements
- Master's Degree in Social Work from an accredited school.
- Certified as a Licensed Clinical Social Worker (LCSW).
- Strong interpersonal communication skills.
- Ability to maintain effective contacts and comfort patients and families.
