SOCIAL WORKER - CARE MANAGEMENT - CARROLL HOSPITAL

Job Description

We are looking for a passionate and experienced Social Worker to join our Care Management team at Carroll Hospital. This part-time position with a weekend commitment offers day shifts from 9:00 am to 5:30 pm. As a Social Worker, you will collaborate with healthcare team members to ensure the efficient care of inpatients and coordinate with community agencies to address various social issues.

Responsibilities

• Partner with healthcare team members for efficient inpatient care.
• Coordinate with community agencies for social issue interventions.
• Address abuse, neglect, substance abuse, mental health, and adoption concerns.
• Link patients to financial resources and support services.

Requirements

• Master's degree in social work.
• 2-4 years of social work experience, preferably in a medical setting.
• LCSW-C license preferred, LMSW licensed candidates considered.
