JOB SUMMARY: Provides clinical social work services or psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, medical understanding, aging changes, discharge planning, and counseling in end-of-life situations.

Requirements

• Basic professional knowledge equivalent to a Bachelor's degree
• 1-3 years of experience
• LBSW/LMSW/LGPC certification
