Job Description
We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a range of common challenges. Our ideal candidate will have a Bachelor's degree, 1-3 years of experience, and hold LBSW/LMSW/LGPC certification.
Responsibilities
- Offer support to patients and families facing adjustment difficulties and various emotional challenges.
- Assist in financial concerns, family relationships, and end-of-life counseling.
- Collaborate with healthcare teams to develop discharge plans and ensure continuity of care.
- Provide compassionate counseling related to death and dying.
Requirements
- Bachelor's degree or equivalent in a related field.
- 1-3 years of relevant experience in social work.
- LBSW/LMSW/LGPC certification.
- Strong interpersonal and communication skills.
