SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As part of the Social Work-Hospital team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face during their healthcare journey.

Responsibilities

• Offer support and counseling to patients and families on a range of issues including adjustment to institutional living, financial concerns, family relationships, and end-of-life planning.
• Conduct psychosocial assessments of patients to identify needs and develop appropriate care plans.
• Collaborate with healthcare teams to facilitate discharge planning and ensure continuity of care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in clinical social work.
• Licensed Social Worker (LBSW, LMSW, or LGPC).
• Strong communication and interpersonal skills.
