Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our esteemed team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various common challenges such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and addressing end-of-life concerns.
• Offer support for patients coping with medical diagnoses, treatments, and aging-related changes.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
