Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Offer psychosocial assessments to patients and families.
• Provide counseling on various topics like discharge planning and death and dying.
• Work collaboratively with healthcare professionals to address the needs of patients and families.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
