Job Description
We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services and psychosocial assessments to patients and families.
- Address common challenges faced by patients, including adjustment to institutional living, financial concerns, and counseling related to death and dying.
- Assist with discharge planning and provide support as needed.
Requirements
- Bachelor's degree or equivalent professional knowledge.
- 1-3 years of experience in a similar role.
- LBSW, LMSW, or LGPC certification.
