Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your responsibilities will include addressing various common problems such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.

Requirements

• Basic professional knowledge equivalent to a Bachelor's degree.
• 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
