SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various common problems such as adjustment issues, financial concerns, family relationships, and counseling related to medical procedures and end-of-life care.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and coordination of care.
• Address emotional and social needs of patients and their families.
• Collaborate with healthcare professionals to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
