SOCIAL WORKER PRN

Job Description

We are currently looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments, addressing a range of patient and family needs. You will play a crucial role in supporting patients through various challenges and transitions.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and their families.
• Address issues related to institutional living, financial concerns, family dynamics, understanding medical procedures, aging, and end-of-life care.
• Collaborate with healthcare teams to develop discharge plans and provide support during the grieving process.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong interpersonal and communication skills.
