SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges and concerns they may face.

Responsibilities

• Offer clinical social work services to assist patients/residents and families with adjustment and coping strategies.
• Address financial concerns, family relationships, and other psychosocial issues.
• Provide counseling related to medical diagnoses, treatment understanding, and end-of-life care.
• Assist with discharge planning and support patients in transitioning.

Requirements

• Hold a Bachelor's degree and possess 1-3 years of relevant experience.
• Must be a Licensed Bachelor Social Worker (LBSW), Licensed Master Social Worker (LMSW), or Licensed Graduate Professional Counselor (LGPC).
