SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing a wide range of common challenges. This role involves supporting individuals in adjusting to institutional living, managing financial concerns, navigating family relationships, coping with medical conditions, and assisting with discharge planning and end-of-life counseling.

Requirements

• Bachelor's degree with 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
