Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and their families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and end-of-life counseling.

Requirements

• Basic professional knowledge with a Bachelor's degree or equivalent.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
