Job Description
We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients/residents and families. Your responsibilities will include addressing various common problems such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services and psychosocial assessments to patients and families.
- Address various challenges including adjustment to institutional living, financial concerns, and family relationships.
- Offer counseling related to medical diagnoses, aging, powerlessness, and end-of-life care.
- Assist in discharge planning for patients.
Requirements
- Bachelor's degree and 1-3 years of experience in social work.
- Licensed as LBSW, LMSW, or LGPC.
- Strong knowledge of social work theory and practice.
