Job Description
We are looking for a compassionate and skilled Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position, on an as-needed basis, involves providing clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a range of common issues including adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to address various patient and family concerns.
- Aid in discharge planning and navigate patients through difficult medical decisions.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
Requirements
- Bachelor's degree or equivalent with 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Knowledge of social work theory and practice.
