SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker (PRN) to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges and needs.

Responsibilities

• Conduct psychosocial assessments and provide counseling to address emotional and social aspects of patient care.
• Assist patients and families in coping with institutional living, financial concerns, and changes related to health and aging.
• Collaborate with the healthcare team to develop discharge plans and provide support related to end-of-life care.

Requirements

• Bachelor’s degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW/LMSW/LGPC.
• Strong knowledge of social work theory and practice.
