SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD, specifically at Levindale Hospital. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and their families. You will address a variety of common issues including adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common problems such as adjustment to institutional living and family relationships.
• Offer counseling related to death and dying.
• Assist with discharge planning.

Requirements

• Basic professional knowledge with 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
