SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide essential clinical social work services and psychosocial assessments to patients/residents and their families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common problems faced by patients/residents and families.
• Conduct discharge planning and provide counseling on various issues.

Requirements

• Basic professional knowledge equivalent to a Bachelor's degree.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification.
