SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services to patients/residents and families, addressing various challenges including adjustment to institutional living, financial concerns, family relationships, and discharge planning. Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address issues related to aging, end-of-life care, and other emotional challenges.
• Collaborate with healthcare professionals to develop comprehensive care plans.
• Ensure patients' mental and emotional well-being throughout their healthcare journey.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a clinical setting.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
