Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker PRN, you will provide essential clinical social work services and psychosocial assessments to patients, assisting them with various challenges they may face. Responsibilities

• Provide clinical social work services to patients and their families.
• Assess and address common problems such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
• Support patients in understanding and coping with medical diagnoses and procedures.
• Participate in discharge planning and provide guidance on issues related to aging.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience as a Social Worker.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
