Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various emotional and practical challenges they may face during their healthcare journey.

Responsibilities

• Conduct psychosocial assessments and provide counseling to address emotional, financial, and family-related issues.
• Assist patients with adjustment to institutional living and coping with medical diagnoses and procedures.
• Collaborate with interdisciplinary teams to develop discharge plans and ensure smooth transitions for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience with LBSW/LMSW/LGPC certification.
• Ability to apply professional knowledge in a healthcare setting with empathy and sensitivity.
