SOCIAL WORKER PRN

Job Description

We are looking to hire a dedicated Social Worker (PRN) to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide psychosocial assessments to patients and families.
• Offer counseling and support for a range of emotional and social challenges.
• Assist with discharge planning and end-of-life care.
• Collaborate with healthcare teams to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong interpersonal and communication skills.
