SOCIAL WORKER PRN

Job Description

We are currently seeking a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a range of common concerns such as adjustment to institutional living, financial issues, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling services.
• Assist patients with discharge planning and transition to care facilities.
• Collaborate with medical professionals to address the holistic needs of patients.
• Offer emotional support and guidance to individuals and families in challenging situations.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Solid understanding of social work theory and practice.
