SOCIAL WORKER PRN

Job Description

We are looking for a dedicated SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD, specifically at Levindale Hospital. In this role, you will provide clinical social work services and psychosocial assessments to patients, assisting them and their families with various challenges such as adjustment difficulties, financial concerns, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist patients with adjustment to institutional living and aging-related changes.
• Offer support for financial concerns and discharge planning.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience and LBSW/LMSW/LGPC certification.
• Knowledge of theory and practice in social work.
