Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Conduct psychosocial assessments to address common challenges.
• Assist with adjustment to institutional living and discharge planning.
• Offer counseling related to aging, death, and dying.

Requirements

• Bachelor's degree with 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
