SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Conduct psychosocial assessments and provide counseling.
• Assist patients/residents and families with issues related to aging, medical diagnoses, and relationships.
• Collaborate with healthcare teams to develop discharge plans.

Requirements

• Bachelor's degree and 1-3 years of experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
• Excellent communication and interpersonal skills.
