SOCIAL WORKER PRN

Job Description

We are hiring a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and their families.
• Handle common problems related to aging, family relationships, and end-of-life discussions.
• Assist patients in understanding and accepting medical conditions and treatment procedures.
• Participate in discharge planning and counseling sessions.

Requirements

• Bachelor's degree and 1-3 years of experience.
• Licensed as LBSW, LMSW, or LGPC.
• Sound knowledge of clinical social work theory and practice.
