SOCIAL WORKER PRN

Job Description

We are currently looking for a PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, families, and residents, addressing a wide range of common issues. The position involves working with patients on adjustment to institutional living, financial concerns, family relationships, aging-related changes, powerlessness, discharge planning, and counseling on end-of-life matters.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of experience as a social worker.
• LBSW, LMSW, or LGPC certification.
