Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Offer clinical social work services to patients and families.
• Conduct psychosocial assessments and provide counseling.
• Assist with discharge planning and end-of-life support.

Requirements

• Bachelor's degree in social work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
