SOCIAL WORKER PRN

Job Description

We are looking for a dedicated and compassionate Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As a PRN Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges related to institutional living, financial concerns, family dynamics, health issues, aging, discharge planning, and end-of-life support.

Responsibilities

• Conduct psychosocial assessments to understand the needs of patients and families.
• Provide counseling and support to individuals coping with medical conditions and end-of-life issues.
• Collaborate with healthcare teams to develop and implement comprehensive care plans.
• Assist with discharge planning and community resource referrals.

Requirements

• Bachelor's degree in Social Work or related field.
• LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in a healthcare setting.
• Strong interpersonal and communication skills.
