Job Description

We are looking for a dedicated and compassionate Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing a range of common issues related to institutional living, financial concerns, family relationships, medical treatments, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address patient and family concerns related to various life challenges.
• Collaborate with healthcare teams to support patient well-being.
• Ensure effective discharge planning and counseling.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
