SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health, located in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Provide counseling and support to address emotional, financial, and family-related challenges.
• Collaborate with healthcare teams to develop discharge plans.
• Offer guidance on end-of-life care and bereavement support.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
• Strong interpersonal and communication skills.
