SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common problems faced by patients/residents and families.
• Offer counseling related to death and dying.
• Collaborate with the healthcare team for optimal patient care.

Requirements

• Bachelor's degree and 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
• Demonstrated knowledge of social work theory and practice.
