SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker (PRN) to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. As a vital member of the Allied Health department, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family dynamics, end-of-life issues, and more.

Responsibilities

• Conduct psychosocial assessments and develop care plans.
• Offer counseling and support to patients and families facing difficult situations.
• Assist with discharge planning and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
