Job Description

We are looking for a dedicated and compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide invaluable clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various common challenges with empathy and expertise.

Responsibilities

• Provide clinical social work services to address adjustment to institutional living, financial concerns, family relationships, and more.
• Offer counseling on medical diagnoses, discharge planning, aging-related changes, and end-of-life support.
• Collaborate with healthcare teams to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
