Job Description

We are looking for a compassionate and dedicated Social Worker for a PRN position at LifeBridge Health in Baltimore, MD. As part of the team at Levindale Hospital, you will provide clinical social work services to patients/residents and families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Conduct psychosocial assessments and provide counseling.
• Assist with discharge planning and end-of-life care.
• Collaborate with healthcare professionals to support patient well-being.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
