SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Provide psychosocial assessments to patients and families.
• Address common problems related to adjustment, financial concerns, and family dynamics.
• Offer counseling on various issues including aging, powerlessness, and end-of-life planning.
• Collaborate with healthcare teams to support patients' well-being.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience as a Social Worker.
• LBSW, LMSW, or LGPC certification.
• Strong clinical knowledge and empathy for patient care.
