SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various concerns related to institutional living, financial matters, family relationships, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Deliver clinical social work services to patients and their families.
• Address common problems such as adjustment to institutional living, financial issues, family dynamics, and end-of-life counseling.
• Collaborate with healthcare teams to support patient care and well-being.
• Conduct psychosocial assessments and provide appropriate interventions.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification.
• Strong knowledge of social work theory and practices.
